Insurance Information

We are contracted with most insurance plans, including Medicare, Regence, Premera, Cigna, Aetna, Humana, Group Health, USFHP, United Healthcare, DSHS, Labor and Industries, Multiplan, First Choice Health Network, and many Medicare Advantage plans. We are also part of most of the Washington State Exchange Insurance products. If you have any questions with regard to your coverage before your procedure, please contact one of our Pre-Authorization representatives, by calling (425) 709-2500. If you have questions about how your insurance processed payment on your account, please call one of our billers at (425) 709-2500.

We have contracts with insurance companies, just as you do. As a reminder, it is always best for a patient to be familiar with their coverage and limitations of their plans before surgery. We are happy to help any way we can.

We will bill all health insurance plans for which you have provided billing information to us.

Please bring all insurance cards and photo ID with you to OSC. Please also bring any deposits, co-payments or co-insurance payments that are required. In addition to cash, checks and money orders, we accept Mastercard, Visa, American Express and Discover cards.

You and/or your insurance companies will be billed for facility services at Overlake Surgery Center. Our charges include the facility, surgery center staffing, medications and drugs used while here at the center, medical supplies, implants and allografts. You will receive a separate bill for any lab work, take-home medications, anesthesia services, your physician, a physician assistant, any blood products, and any charges associated with a hospital transfer.
